WHAT WE DO?
Each consumer has their own bilingual Supports Coordinator that will work with you and your family to develop a relationship based on respect, support and collaboration.
Within this alliance, Supports Coordinators develop individual service plans (ISP) for each consumer. The ISP is based on your strengths and interests as well as identifies specific and personal goals to be achieved.
Here are some of the things that we can expect from us:
⦁Assistance in accessing the proper treatment, other services, natural supports, community resources and opportunities that are specified in the consumer’s ISP;
⦁Help in meeting basic physical needs, and supports that are necessary for a stable, safe, and healthy community life;
⦁Connecting the individual and their family to support networks that are important to the persons we serve.
⦁Preventing and managing crisis through coordination and/or assistance with crisis intervention and stabilization services as appropriate.
⦁Work with consumers to ensure that your voices are heard when dealing with other service agencies, physicians, medical plans, and more. Our hope is that over time, our Service Coordinators will help their consumers become effective advocates for themselves. You will be able say I went from Arise to “I RISE”.
⦁Monitoring the individual service plans and make sure treatments remain appropriate. Service Coordinators help consumers determine whether their service plan is not only meeting their needs, but also working toward their hopes in life.
⦁Coordinating a consumer’s physical and mental health treatments, determine the best methods of finding funding for treatment, help find transportation to appointments, and more.
⦁Completing necessary/appropriate documentation, including maintaining individuals’ eligibility for waiver services, copies of the Individual Service Plan (ISP), and service plan addendums, and other related information.
