Who is eligible to receive Supports Coordination Services through OLTL?
Supports Coordination services can be provided to individuals who qualify for the following:
Aging Waiver – for individuals 60 years of age and older that live in their own homes, with the goal of avoiding institutionalization and that permits them to remain as independent as possible in their own homes.
Attendant Care – for individuals 18-59 years of age that have physical disabilities, to avoid institutionalization and permits them to remain as independent as possible.
Independence Waiver - this program provides services to persons, who have physical disabilities, but the goal is to prevent institutionalization and it allows individuals to live as independently as possible. The beneficiaries need to be 18 years of age or older, have a chronic physical disability such as diabetes, chronic asthma, heart diseases, cancer, hypertension, MS, HIV, etc.
COMMCARE Waiver - individuals need to be 21 years old or older, have a Traumatic Brain Injury (TBI), to qualify for Special Rehabilitative Services (SRF).
